They can be on the same page, but for your SMS setup I’d keep them as two separate pages.
That way you can paste one URL into Privacy Policy URL and a different URL into Terms of Service URL, exactly as the form is asking.

You can add your logo, and I think it would look good at the top of the page.
Keep it simple though: one small-to-medium logo above the Privacy Policy or SMS Terms of Service heading, then leave the rest of the page mostly text. That keeps the page professional and still clearly branded.
I would not add product pictures or decorative images to a legal page. Just the logo is enough.
